Special Education Assessment Request Letter

There are three steps to creating a special education assessment request letter, appropriate for children ages 3-22 years old. [(For children under 3, see our early intervention program page.)]

  1. Look up the address of your local school - look up schools in Santa Clara County or look up schools anywhere in the USA.
  2. Copy and paste the letter below into your own word processing program for replacing the generic fields (those that are in bold) with your personal information and refinement.
  3. Please consider attending one of PHP's Individualized Education Plan (IEP) trainings so that you can become the best advocate for your child!

I am writing to refer my child, [Child's Name], for assessment to determine if he is eligible for special education services. He is 5 years old and currently attends [Home School].

I request that the [My District] District conduct a full evaluation of my child to determine whether he qualifies for special education services under the Individuals with Disabilities Education Act (IDEA). I am requesting the full evaluation for the following reasons: Here is where you list out your concerns. I understand that all areas of difficulty should be assessed and I would like to talk with you about the types of testing that should be completed to obtain the information we need.

I also request that my child be evaluated under Section 504 of the Rehabilitation Act of 1973 for the presence of any educational service need that may require any accommodation or program modification not available under special education or if my child is not found eligible for special education. I also request that the Section 504 Coordinator for [My District] be present at the initial IEP meeting to discuss the results and recommendations of the Section 504 Evaluation.

I look forward to receiving an assessment plan in 15 days of this request. I hope that these evaluations will be completed promptly. Thereafter, we can have an IEP meeting to discuss the results of these evaluations and plan for [Child's Name] continued education. Please ensure that I receive copies of the assessment reports one week before the IEP meeting.
