Family support services:

Family support services are community-based services that build on the strength of families. The programs emphasize the partnerships between agency staff and families based on equality and respect. Programs affirm and strengthen families’ cultural, racial, and linguistic identities and enhance their ability to function in a multicultural society. Staff advocate with families for services and systems that are fair, responsive, and accountable to the families serviced. Programs and services are flexible, family centered, and work to preserve the unit of the family.

Family Support Services
Family Centered Medical Case Management
Sunburst Projects ensures a timely access to and coordination of medical and psychosocial services for families impacted by HIV/AIDS. Our medical case management is aimed at providing individualized care that strengthens the family unit. In addition to assisting clients access and maintain specific health care needs case management activities may include negotiation and advocacy for services, consultation with providers, navigation through the service system, psycho-social support, legal support, and general client education.

Mental Health Services
Professional counseling and psychotherapy services are offered in English and Spanish to individuals, couples, children, and adolescents, receiving medical care. With more than thirty years experience in helping people deal effectively with life’s challenges, our goal is to promote mental health services that strengthen the family as a unit and creates positive change.

HIV Positive Support Groups
Communicating with others who are going through similar life experiences is one of the best ways people can help themselves. People living with HIV/AIDS sometimes have needs greater than what can be addressed in other groups. Sunburst Projects sponsors groups specifically for women, youth, and families impacted by HIV/AIDS.

HIV Education and Prevention
Sunburst Projects promotes HIV prevention through a broad range of community outreach events. Often HIV positive women and youth share basic facts on HIV/AIDS and use their own stories to reinforce the message that young people today must educate and protect themselves.

Youth Services
Sunburst Projects provides residential youth leadership camps, wilderness therapy, One Love conference and peer mentorship programs that foster self-sufficiency, career development and a path to independence.

In an effort to help prepare youth for their transition into adult medical care, Sunburst Projects in partnership with the University of California Davis (UCD) provides a monthly transitional medical clinic for youth 18-24. This setting gives each youth the opportunity to discuss issues about living with HIV/AIDS, and address their concerns on medication adherence and disclosure. It also provides a space where psychological interventions can be developed in preparation for their transition into adult care.

Childcare
Offers childcare vouchers that allow families to attend to their medical and mental health needs. This service supports clients’ desires to enhance their health and overall quality of life.
