• Proof of income (recent paycheck stub, 2010 tax forms, employer letter, affidavit, etc.)
• Proof of residency (i.e. utility bill)
• Child’s birth certificate, social security card or immigration documents (if applicable)

• Spanish/English Translation Provided
• KidZone: Free Supervised Activities for Kids
• Healthy Food Giveaway for First 500 Families
