We believe that one-page descriptions are the foundation of personalization, and can lead to positive change for people, whatever their age or circumstances. They provide us with an at-a-glance way of knowing what really matters to people, that can be taken with them as they move through services and come into contact with people.
One Page Descriptions are easy to read and obtain three critical sections.
1.What people like and admire about the person,
2.What the person defines as important to him/her,
3.How to support the person in a way that works for him/her.
Those that support individuals that use services are informed and therefore, more successful in their role, which may lead to increased longevity.
Learn how to separate information into a reference for caregivers and create a positive introduction for someone.
Lives are changed with knowledge, supports are improved, and people lead happier and more fulfilling lives.
Please register in advance as the room has limited seating. There is a $10 fee to attend. Your regional center may fund your attendance. This service qualifies to maintain a medicaid waiver through the regional center.
