This two-part workshop series will focus on defining, aiming, and designing an Individual Transition Plan (ITP) in the IEP. Starting with the assessment process, linking to self-determination, considering meaningful goals, and providing related services while bringing together mandates of various agencies are the foundations of the ITP; what you need to know as your teen approaches their 16th birthday, and maybe even before that!”

In part one, we will be discussing:

  • What is an ITP?
  • What is the transition age & age of the majority?
  • How to pre-plan & plan for transition.
  • The assessment process
  • Working with your team

Part 2 will focus on developing the transition plan, so do not forget to register for part two on Th, August Wednesday 17 at 6 pm. This workshop will be simultaneously interpreted into Vietnamese.
