This two-part workshop series will focus on defining, aiming, and designing an Individual Transition Plan (ITP) in the IEP. Starting with the assessment process, linking to self-determination, considering meaningful goals, and providing related services while bringing together mandates of various agencies are the foundations of the ITP; what you need to know as your teen approaches their 16th birthday, and maybe even before that!

In part two, we will be discussing:

  • Developing the ITP
  • Goals, services, and supports
  • Diploma Vs. Certification
  • Self-determination
  • Working with your team
