One Page Description of Your Child – Workshop

Create a One Page Description for your child or loved one.

A One Page Description is an amazing tool that communicates, in a simple and visual way, a positively-focused summary of what others admire about your child, what is important to them, and how to best support them. A One Page Description can help teachers, therapists, caregivers, doctors, family members, and others, provide person-centered care and support.

At the end of the session, you will have a completed One Page Description about your child or loved one which you can share widely! You may find yourself using the same person-centered tools you will learn to create a profile for all your children, your parents, grandparents, and even yourself!

Participation is limited to families who live in Santa Clara County with children ages 0 – 17. Attendance is limited to allow for confidential sharing and discussion.
