It also includes what is important to them, how to best support them, and a way to communicate what’s working and not working for them.

A profile can help teachers, therapists, caregivers, doctors, family members, and others provide person-centered care and support.

Once you create a profile, it is so easy to quickly and positively share about your loved one! Teachers, therapists, care and medical staff, community helpers, and more can learn about your loved one from their profile!

You may even find yourself using the same person-centered planning tools to create a profile for all your children, your parents, grandparents, and even yourself!
