Independent Or Supported Living Skills
Job/Vocational
East Bay Innovations provides personalized support to enable individuals with disabilities to live in their homes, to work jobs of their choosing and to participate in all aspects of the community.
Independent Living
Independent Living offers consumers the chance to learn more living skills to gain increased independence.
Independent Living Services instructors teach various skills to consumers, including grocery shopping, planning a meal, making a budget, balancing a check book, getting in shape, dealing with social security and lots more.
Supported Living
Supported Living offers consumers the chance to be able to live independently in their own homes.
Community living assistants, with supervision and support from their supported living facilitators, assist their consumers with all of the skills that the Independent Living Services instructors teach, as well as many times providing personal assistance care and overnight support to those consumers who cannot be left alone but still want to be independent.
Employment Services
Employment Services offers consumers a chance to work within their communities. Job developers find positions suitable for individual consumers.
Job coaches train consumers to enable them to do their jobs. This includes but is not limited to: travel training to and from the job, proper grooming and dressing techniques, and on-the-job training.
Project SEARCH
Project SEARCH offers consumers the opportunity to develop valuable and transferable work skills through an on-the-job vocational training program and job placement program.
This program is offered through the joint efforts of Oakland Adult and Career Education Adults with Disabilities Program, College of Alameda and East Bay Innovations. Internships take place at UCSF Benioff Children’s Hospital Oakland and the County of Alameda. Project SEARCH job coaches work with consumers to help them successfully complete their internship and gain experience to build their resume.
Community Day Support
Community Day Support is a community based day service where individuals who have significant physical disabilities or intensive medical needs an receive individualized support to participate in variety of activities.
Community Living Assistants support individuals to participate in activities such as adaptive communication instruction, physical therapies, independent living skills instruction, art classes, movement/dance classes and fitness activities.
California Community Transitions
California Community Transitions makes it possible for people with disabilities who receive Medi-Cal to leave nursing facilities and move into their own homes.
Transition specialists provide case management, search for affordable housing, help consumers access the In-Home Supportive Services Program, assist in training personal attendants, and identify needed healthcare professionals in the community.
LifeTrust Care (LTC) is a collaborative of three Bay Area nonprofit organizations with extensive expertise providing services to individuals with disabilities and seniors: East Bay Innovations, Integrated Community Services, and Bayberry Inc. LTC staff provide tailored support to meet each client’s unique needs, increase stability, and improve their quality of life.
