Interventional Psychiatry

Interventional Psychiatry
Interventional psychiatry is a mental health specialty that uses electrical or magnetic stimulation, as well as some drug treatments, to alter brain circuitry and subsequently ease the symptoms of long-term depression, bipolar disorder and anxiety disorders.

You may be a candidate for interventional psychiatry if medication and therapy have not helped you.

Currently, electroconvulsive therapy is offered at all Sutter behavioral health centers, while other interventional psychiatry procedures are offered at the Sutter Center for Psychiatry in Sacramento and Alta Bates Summit Medical Center in Oakland and Berkeley.

Electroconvulsive Therapy (ECT)

Electroconvulsive therapy is a procedure that passes a small electric current through your brain to treat long-term depression and other serious behavioral health illnesses. In recent years, extensive research around ECT has confirmed it’s a safe and effective treatment for significantly reducing the symptoms of severe depression, mania and catatonia.

ECT takes about five to 10 minutes, with added time for preparation and recovery. For your first treatment, you will probably be admitted to the hospital. You may be able to have future treatments on an outpatient basis.

While you are under anesthesia, a small amount of electric current is delivered to your head, causing seizure activity in your brain. This process lasts for about 40 seconds. Doctors believe the seizure activity may help the brain “rewire” itself, which can relieve symptoms.

Vagal Nerve Stimulation (VNS)

Vagal nerve stimulation uses electrical impulses to stimulate areas of the brain that regulate mood, sleep, appetite and motivation. It is used to treat long-term or recurring depression and bipolar disorder.

Subtle electrical signals are delivered to the brain from a battery-operated, computer-controlled device called a pulse generator, which is surgically implanted in the upper chest like a pacemaker. Wires attach the device to the left vagus nerve, which travels from the brain down through the neck and to several of the major organs. Electric signals travel from the generator to the vagus nerve. When stimulated, the nerve sends signals up to the brain.

These electrical impulses relieve symptoms of depression or mania in some people–for reasons that doctors don’t fully understand. It usually takes several months, however, to see the results.

Transcranial Magnetic Stimulation (TMS)

Transcranial magnetic stimulation uses short pulses of magnetic fields to stimulate nerve cells in the area of the brain thought to control mood. These pulsed magnetic fields may have a positive effect on the brain’s neurotransmitters levels. TMS may provide an alternative treatment option for those who have not benefited from the use of antidepressant medication.

TMS is an outpatient procedure done in your psychiatrist’s office, and does not require sedation or surgery. You remain awake and alert during treatment, which lasts about 20 to 40 minutes. TMS consists of at least five treatments a week during a four to six-week period.

Ketamine Infusion Therapy (KIT)

Ketamine infusion therapy is a newer treatment option for people with mental health conditions that have not improved with medications or other interventional procedures.

The drug acts on an entirely different neurotransmitter system than antidepressants, and often brings rapid relief to some people with severe depression or suicidal thoughts.

When you arrive for your treatment, you are placed on a bed, and an IV is inserted in your arm. You then receive a 40-minute infusion of a very low dose of ketamine. Though this drug is an anesthetic, the dose is so low that at most, you may become a little drowsy.

Suspecting your child has a developmental delay or a difference can be scary and confusing. Many families who visit us don’t know how to help their children or understand what conditions their child may have.

The experts at CPMC’s Kalmanovitz Child Development Center provide comprehensive developmental assessment and treatment programs for infants, preschoolers, school-age children and adolescents who manifest developmental delays or differences. We also help children with other special needs. A multi-disciplinary team assists families and professionals with recommendations about the child/adolescent’s developmental, behavioral and learning needs by providing diagnostic information, treatment options, consultation, continuity of service and support for families.

In existence for more than 50 years, we are the largest multi-disciplinary clinic of this kind in Northern California, providing about 18,000 visits per year. Our highly trained and caring child development experts are committed to meeting the needs of you and your child. The Center has bilingual staff in Spanish for most services, and interpreter services are available in a variety of languages for all services.

Adolescent Services — For adolescents and young adults ages 12 to 21, we provide educational testing and therapy, psychological testing and therapy, assessments to diagnose learning disabilities, and consultations regarding college choices and admission testing. Integrated Social Skills Playgroups — Under staff supervision, integrated playgroups are formed to further enhance a child’s social, language, and motor skills utilizing speech and language, occupational therapy and behavioral therapy. Early Intervention Program/GGRC — This early intervention program for children at risk for developmental disorders is supported by Golden Gate Regional Center. It assists children in many areas of development and teaches parents how to advance skill development. Children ages 5 months to 3 years spend two hours once a week in a small group, preschool-like setting. The program is staffed by an infant educator, a speech and language pathologist, a physical therapist and an occupational therapist.
Parents participate in the support group and have access to information, resources and referrals. Each family is also eligible for three home visits a month. Families also have the option of participating in a home visit only program, where the child is seen for four to five visits a month. At age 3, children are transitioned to the school district. Psychological Assessment and Treatment — We provide evaluations of cognitive, academic and emotional development; short-term individual and family psychotherapy and consultations on behavior management and child rearing issues. Call (415) 600-6200 for more information.
School Age Learning Program — For children ages 6 to 12, we offer assessments of educational achievement, cognitive development, possible learning issues, learning aptitude, handwriting skills and psychosocial adjustment. Bringing greater independence to those with vision loss

Overview
Despite new technology and continuing research, partial or total vision loss remains a problem for millions of Americans. For these individuals, everyday tasks such as reading, shopping and recognizing faces are difficult because their vision impairment leaves them in a “gray zone” between normal eyesight and blindness.

Some of the most common causes of low vision include:

Macular degeneration
Diabetic retinopathy
Glaucoma
Stroke or brain injury
With low vision, one’s eyesight often cannot be improved with regular glasses, medicine or surgery. Therefore, vision rehabilitation is necessary so individuals can learn compensatory visual skills, such as shifting images to a different part of the retinal anatomy. This training, combined with low-vision devices, can help one safely and independently complete daily activities that are now difficult because of vision loss.

Vision Enhancement
Many devices are available to help people with low vision function better. Our team will prescribe the best devices for your impairment, whether it is seeing objects at a distance, reading, viewing television or writing. Common devices or techniques that help make everyday activities easier include:

High-contrast and large-number telephones, watches and remote controls
Computers that can magnify printed material or pictures
Talking watches, timers and medical equipment
Bold-tipped markers for easy-to-read shopping lists
